ECE Forms
On-campus non-thesis MS students in ECE may participate in a full-time internship during a spring or fall semester(minimum of 4 months) and receive 3 credit hours of ECE 650 by completing the process below. Student must be in good academic standing (GPA>3.0) to participate. ECE 650 is a course that requires an internship with a company or organization outside NCSU. It is intended for non-thesis Master’s students and can only be taken once. During the semester of the internship, the student will be responsible for enrolling and paying tuition for ECE 650 and COP 500 and will be considered a full-time student. Step 1: Submit plan of work on MyPack
A plan of work needs to be submitted on MyPack as a requirement to be approved for the full-time internship during a fall or spring semester. Only 3 credit hours of S/U courses can be included in the plan of work for non-thesis Master’s students. (excluding S/U grades for COVID semesters) The plan of work must show that the student will finish the non-thesis MS in 2 years, as a program extension will not be allowed for international students for this internship.
Include ECE 650 in the semester you will complete the internship and include any planned courses to meet degree requirements. Please list me (Donna Yu) as the chair of your committee and it will get routed to me for approval. If you need to make changes to your plan of work, please email your student ID number to ecegradoffice@ncsu.edu with your request to reset your plan of work.
Step 2: CPT (Curricular Practical Training) Application for International Students
- International students will have to meet the requirements for CPT to be eligible for ECE 650.
- Review the eligibility requirements and process to apply for CPT with OIS (Office of International Services.
Guidelines on CPT start and end dates:
- Student must be in good academic standing (GPA>3.0)
- ADD the Internship subplan (14CPEIMS, 14EEIMS or 14CNEIMS) to your degree program (if the option isn’t listed in the dropdown box, please write this in the justifications box)
- The Employment offer letter should include Job Title, Job Description, Salary, Job hours/week, Physical Address of Building of employment (in body of letter, not letterhead), Start Date and End Date.
- Please list Ginger Yu as the advisor and include ecegradoffice@ncsu.edu for the email address so that it will be routed correctly for approval.
- OIS will not approve the CPT until the Internship sub-plan has been approved and the student has been enrolled in ECE 650.
CPT is authorized on a semester-by-semester basis. Each semester of CPT needs a separate application. Please see the NCSU Three Year Calendar.
Spring semester:
Earliest start date: Day after fall ‘Commencement’ (aka graduation)
Latest end date: Day before ‘First Day of Classes’ in the 1st summer session*
Summer semester:
Earliest start date: Day after spring ‘Commencement’ (aka graduation)
Latest end date: Day before ‘First Day of Classes’ in the fall semester*
Fall semester:
Earliest start date: Day after 2nd summer semester ‘Final Examinations’ end
Latest end date: Day before ‘First Day of Classes’ in the spring semester*
*unless the student is completing degree requirements during that semester
Step 3: Internship Form
Complete the Internship Form to:
- Give permission to add the Internship subplan (14CPEIMS, 14EEIMS or 14CNEIMS) to your degree program.
- Give permission to be enrolled in ECE 650 – Ginger Yu will be listed as the instructor.
- ECE 650 Evaluation Criteria: A meeting with the ADGP to discuss the required written two-page report documenting the internship, which includes: i) A description of the job activities and responsibilities. If this is not your first work semester, describe only your new activities or how you have expanded your responsibilities. ii) The relation of your co‐op experience to your academic studies. iii) The most and least satisfying aspects of your co‐op experience. A student self assessment will be completed during the meeting that addresses the student’s attitude towards work, ability to learn, initiative, quality of work, quantity of work, teamwork, and communication skills.
- Indicate if your internship will be remote or in-person
- Request enrolling in an additional course while working full-time.
Please complete these 3 steps simultaneously. Information from all of these steps is needed to give final approval for the internship.
Submit an enrollment request form for Research Credits, Individual Studies, Qualifying Review Credits, and Special Topics. The form will then be routed to the instructor for approval and the ECE Graduate Office will then enroll you into the requested course within 3-5 business days of instructor approval.

In order to apply to the PhD Program 2 different faculty members must sign the ‘Application for Admissions’. All students applying to the ECE PhD program must also submit the Graduate School’s “Change of Degree Status or Curriculum” form. International students must also submit an OIS ‘Change of Level/Curriculum’ form.
Students who enter the PhD program with a previous MS degree must take the Qualifying Review no later than the third semester of PhD enrollment. Students who enter the PhD program without a previous MS degree may delay the Qualifying Review to the fourth semester of PhD enrollment. This must be approved by the student’s Qualifying Review Committee.
Purpose of Exam & Scheduling Details:
Our Qualifying Review Exam is typically only given to PhD students and is used to:
- Measure the student’s breadth of knowledge by his/her performance in at least three graduate level courses that are outside the student’s specialty.
- Determine the in-depth knowledge
- Determine the suitability of the student to perform competent research.
This exam does not have to be scheduled formally through the Graduate School (like the Preliminary and Final Doctoral exams do). Your Graduate Advisory Committee decides the exact nature of the Qualifying Review. The Qualifying Review may take many forms; however, it must have both oral and written components. Please discuss this with your PhD advisor to begin preparing for the exam.
A week before the exam, we ask that either you or your advisor email our office with the following details to schedule your Qualifying Review Exam:
- Student’s Name
- 9-digit student ID number
- Date of the Exam
- Names of Faculty Members on the Committee

A change of degree form is required to, change from one MS program to another, to change from MS to PhD, or to add an En Route MS.
Once you have submitted all of the required forms we will get the required signatures. We will then send you a copy of the OIS form which you can then submit to OIS directly, this will update your I-20. We will forward the Graduate School form directly to the Graduate School to finish processing. Once the Graduate School has processed the request you will notice the change reflected in your mypack portal.
PhD – Request to Schedule Prelim/Final Examination
MS Thesis – Request to Schedule Final Oral Examination
This form must be submitted 2 weeks prior to exam date.
Before submitting please be sure of the following:
- Applied to Graduate
- Plan of Work has been approved
- Committee members on form match committee members in PoW
- Have passed the Prelim OralExam
If you need to make changes to your committee members please submit the Committee Modification Form
If you have an external committee member please submit the Advisory Committee Appointment of External Member or Technical Consultant Form
Required for all students needing to take up to 2 semesters off. Please be sure when submitting the form you have dropped all course you may have enrolled in for the semester(s) you are requesting the leave of absence.
If either you or a significant other is expecting/adopting please submit the Parental Leave Request form along with the certification form to completed by your health care provider.

Required for all students looking to go over the max number of credit hours during a semester or make changes to their schedule after Census Day. Please submit the schedule revision form on MyPack. Student instructions on completing the form can be found HERE.
Required of all students to transfer credit from other institutions and other NCSU programs, including PBS.
If transferring credit from other institions please be sure to include
- a copy of your transcript
- a copy of the syllabus

All students submitting an OPT Application must have
- Plan of Work approved
- Applied to graduate through your MyPack Portal
If you are a MS Thesis/PhD Student you must have
- Scheduled your final defense
- Your adviser approve and sign the OPT form before submitting to the ECE Grad Office.
If you are or will be an RA/TA in the semester the OPT will start
- You must select which day you would like your RA/TA appointment terminated on
- Thesis Defense date
- Graduation date
- Submit the Program Extension Form
- Select one of the Items listed under the “circumstances and academic reason for delay…”
- Submit a brief explanation of the delay from your adviser along with your Program Extension Request for DGP approval
- Have your adviser complete a funding approval letter to submit with your Program Extension Form
